Health Information Management

The Health Information Management Department consists of these functional areas:

  • Release of patient information (Guidelines)
  • Patient Registration
  • Accessibility and Diversity
  • Provision of statistical information
  • Provision of information for patient care
  • Medical transcription

Each functional area supports and contributes to patient care.

Guidelines for Access to Patient Information :

To access patient information in accordance with legislative requirements we require a written authorization containing the following:

  • The authorization must be directed to The Credit Valley Hospital
  • The name of the patient
  • Patient's date of birth
  • Reason for the request
  • The type of information required and treatment dates if possible
  • The name and address of the person you wish the information sent to
  • The authorization must be signed, dated and witnessed

The authorization must be signed by the patient/substitute decision maker. An administrative fee will apply if the requested information is being used for non clinical reasons. The authorization is valid for three months.
